MPI Toronto Chapter Fundraising Raffle Donation Form
I wish to donate a raffle prize item for the Winter Social on December 2, 2026 at Liberty Grand.

Please provide a description of item(s) and list any restrictions, otherwise it will be considered a no-restriction donation.

The event date is December 2, 2026 - gift certificates should have an expiry date of at least 6 months after the event date.

To be included as a raffle donor, donations should be received no later than Friday, November 20, 2026.
